Abstract

Breast cancer is one of the most common cancers among women worldwide and remains a leading cause of cancer-related mortality. Early detection through Breast Self-Examination (BSE) is a simple, low-cost, and effective method for identifying breast abnormalities at an early stage. However, the knowledge and attitudes of adolescent girls toward BSE remain relatively low, highlighting the need for innovative health education using audio-visual media. This study aimed to determine the effectiveness of audio-visual video-based education on Breast Self-Examination (BSE) in improving the knowledge and attitudes of female adolescents at Darussalam Health Vocational High School, Lhokseumawe. This study employed a pre-experimental design using a one-group pretest–posttest design. The study population consisted of all female students at Darussalam Health Vocational High School, Lhokseumawe, with a total sample of 51 respondents selected using the total sampling technique. Data were collected using validated questionnaires measuring knowledge and attitudes regarding BSE, administered before (pretest) and after (posttest) the audio-visual video-based educational intervention. Data were analyzed using univariate analysis with frequency distributions and percentages, while bivariate analysis was performed using the Wilcoxon Signed-Rank Test with a significance level of α = 0.05. The results showed that before the intervention, most respondents had poor knowledge (56.9%) and negative attitudes (68.6%) toward BSE. Following the audio-visual educational intervention, the proportion of respondents with good knowledge increased to 72.5%, while those with positive attitudes increased to 82.4%. The Wilcoxon Signed-Rank Test revealed a statistically significant effect of the audio-visual educational intervention on both knowledge (Z = −6.142; p < 0.001) and attitudes (Z = −5.871; p < 0.001) regarding BSE among female adolescents. It can be concluded that audio-visual video-based health education is effective in improving knowledge and fostering positive attitudes toward Breast Self-Examination (BSE) among female adolescents. Therefore, audio-visual media are recommended as an effective health promotion strategy in school settings to increase adolescents' awareness and participation in the early detection of breast cancer.
